Plus: there was no protest before the attack

An independent inquiry into the Benghazi attack has just been released, and it isn't good news for the State Dept. The report concludes that departmental failures led to "grossly inadequate" embassy security.
"Systematic failures and leadership and management deficiencies at senior levels within two bureaus of the State Department resulted in a Special Mission security posture that was inadequate for Benghazi and grossly inadequate to deal with the attack that took place," said the report issued by the Accountability Review Board.
The report condemned the Bureau of Diplomatic Security and the Bureau of Near East Affairs for disorganization in protecting the consulate. But it didn't single out any officials for disciplinary action.
Sec. of State Hillary Clinton has admitted that her department dropped the ball: She's "accepted 29 suggestions to improve embassy security," according to Fox News. And she acknowledged the report's claim that no protest ever occurred before the embassy was attacked.
